Facility Assessments

SMART Healthcare Services provides Facility Assessments to assist clients in identifying the precise needs for rehabilitation services, as well as the status and well-being of current programs. This detailed assessment identifies areas of strength and weakness, so that rehabilitation service opportunities can be clearly defined. 

FACILITY ASSESSMENTS INCLUDE:

• Pre-Visit - Review of information from the facility to assess the utilization patterns and financial health of the rehabilitation program

• On-Site Visit - Interviews are conducted with department heads to evaluate:
  1. Facility systems for interdisciplinary communications and service delivery
  2. Therapists' involvement in the overall process of the building
  3. Therapy space, equipment and supplies
  4. Specific therapy services offered and involvement in restorative programs, staffing levels, adequacy of documentation and denials management
  5. Compliance with state and federal regulations, payor mix, facility charge structure

• Post Visit - A written narrative is prepared which summarizes pertinent issues involving the delivery of rehabilitation services at the facility
